The following antimicrobials are restricted to Infectious Diseases approval:

  • ampicillin-sulbactam (high dose) 
  • amphotericin B deoxycholate 
  • amphotericin B liposomal
  • artesunate
  • aztreonam
  • bedaquiline 
  • ceftaroline
  • ceftazidime-avibactam
  • ceftolozane-tazobactam
  • chloramphenicol
  • colistimethate
  • daptomycin
  • ertapenem (not restricted in ICU for first 72 hours)
  • fidaxomicin
  • fosfomycin PO
  • imipenem-cilastatin
  • isavuconazole
  • letermovir
  • meropenem (not restricted in ICU for first 72 hours)*
  • micafungin**
  • moxifloxacin
  • polymyxin B 
  • posaconazole**
  • quinidine
  • quinine
  • tedizolid
  • tigecycline
  • voriconazole**

*or ED patients with active ICU admission orders

**Does not pertain to antifungal prophylaxis per Stem Cell Transplant, Heme, Liver, and Lung Transplant protocols 

Does not pertain to antimicrobials in pre-approved order sets (e.g. peri-operative order sets)
