Come up with an idea for a proposed Care Pathway and form a group of content experts.
Step 2:
Email Chris Roach with your proposal
Step 3:
Once your proposal is approved, you will be assigned a Core Consultant to help facilitate the CPG development process including
- Process map
- Brief summary (250 words)
- Supporting literature
- Health equity considerations
- List of all resource requirements associated with CPG
- Suggested success tracking metrics
Step 4:
Present your final Care Pathway to the VUMC Care Pathway Subcommittee for approval/feedback
Step 5:
Partner with the Quality and Safety Data Liaison to build data metrics
