2019 County-Specific Summaries
The 2019 Rutherford and Williamson County summaries were drawn from the VUMC board-approved CHNA. The Davidson County summary is the Healthy Nashville Community Health Assessment, which is a Davidson County-specific report prepared by Metro Public Health Department in collaboration with VUMC and Ascension Saint Thomas Health.

2019 Community Health Summits
VUMC'S Community Health Equity Team hosted a Community Health Summit in each of the three counties for the 2019 cycle - Davidson, Rutherford, and Williamson County. During the summits, the team presented data for the 2019 cycle of the CHNA and facilitated exercises to prioritize health needs. Community Health Improvement Plans
Each county health department is responsible for developing a Community Health Improvement Plan (CHIP)- which is an action-oriented plan outlining the priority community health issues and the strategies for addressing them.
