First Visit
Your child has been referred to our clinic for surgical evaluation and/or treatment. At the time of your visit you will be informed of your child's surgical and treatment needs. Upon arrival you will be asked to sign the patient's name in the roster and sit down in the reception area until the receptionist calls your name for the appointment check-in. Our waiting area is limited, so we ask that you limit the number of family or friends accompanying the patient to two individuals.
Before Your Visit
If you child has had recent testing such as laboratory results, radiology films/reports or clinical history relating to your child’s diagnosis, pleave provide this documentation, particularly x-ray films, for the pediatric surgeon to review before or at the time of surgical consultation. This documentation is vital to your child’s treatment. Your child's records may be obtained from your primary care physician’s office or from the medical institution where the records originated. Our department accepts records by postal mail, fax, or hand-delivery.
During Your Visit
This may be your child’s first time to visit the Vanderbilt Children’s Hospital; please provide the following items upon appointment check-in:
- Social Security card
- Information about closest relative (for example, name, address, phone number
- Insurance card
- Referral or order sheet from primary care physician
- Medical records, lab work, and x-rays
- Advanced Directives or Power of Attorney (If you have these, please them bring with you. You may obtain a blank copy by contacting our Admitting Department at (615) 343-9962.
All Visits
If Running Late
Please contact our office if you are running late for your scheduled appointment. If you are more than 15 minutes late, it may be necessary to reschedule your visit.
Your Wait
The waiting area is equipped with toys, magazines, a Nintendo 64 and a VCR to watch taped movies for the children to enjoy while waiting to see a surgeon. Parental supervision is required in the waiting area.
What To Bring
Outside clinical documentation, including laboratory results, radiology films/reports, or clinical information requested upon your return visit. Also remember to bring new insurance information.
After Your Visit
Should your symptoms worsen or you have questions regarding surgery, follow-up or medication, please call our office. We have office staff and nurses available to answer your questions during normal business hours. If your child's symptoms should worsen during the time our office is closed, we have an emergency telephone line (615 936-1056) available. Please feel free to contact us for any reason.
