The VHAN Care Management awareness campaign, which concluded last week, was devoted to showing how Care Navigators take a holistic approach to helping patients reach their health goals and navigate the complexities of the health care system. The campaign included the rollout of a new RedCap digital patient referral form to make it easier for providers to ask for Care Management assistance.
