Vanderbilt Medical Group is a participant in the Connected Care of Middle Tennessee Accountable Care Organization (ACO) alongside NorthCrest Physician Services and Williamson Medical Group. Launched in January 2018, Connected Care of Middle Tennessee brings together physicians, hospitals and other healthcare providers to give coordinated, high-quality, patient-centered care to their Medicare patients.
A valuable part of participation in the ACO is the opportunity for clinicians and practices to engage with one another and share best practices to provide better health for individuals and populations through improved quality at a reduced cost.
The Vanderbilt Health Affiliated Network ACO serves as the Management Services Organization (MSO) for Connected Care of Middle Tennessee, Connected Care of East Tennessee, Connected Care of West Tennessee, and Connected Care of Mississippi, all of whom are participating in the CMS Medicare Shared Savings Program (MSSP).
