Ionizing Radiation Medical Events are required to be reported to the State of Tennessee by OCRS per TN SRPAR 0400-20-05-.145.
Examples of ionizing radiation medical events are:
RADIOACTIVE MATERIALS (i.e., Radiopharmaceuticals & LDR Brachytherapy)
- Infiltration/Extravasation of a radiopharmaceutical
- Incorrect radiopharmaceutical administered to patient
- Radiopharmaceutical administered via wrong route
- Radiopharmaceutical administered to wrong patient
- Incorrect dose or dosage administered to patient
- Incorrect radionuclide used to treat patient
THERAPEUTIC MACHINE (i.e., HDR Brachytherapy & LINAC)
- Incorrect radionuclide used to treat patient
- Incorrect dose or dosage administered to patient
- Incorrect treatment site used
- Wrong patient treated
- Incorrect dose per fraction used
- Incorrect number of fractions delivered
