Overview of VUMC Community Health Needs Assessment

The Community Health Needs Assessment (CHNA) process is designed to identify key health needs and assets through systematic, comprehensive data collection in target communities and serves as a health profile for the community in which VUMC patients live. The CHNA describes the significant health needs identified in collaboration with the community, as well as gaps between current and desired health status, and broad multi-sectoral perspectives on health and healthcare - with a focus on historically marginalized communities. VUMC serves individuals and communities across the southeast and from around the world. However, a large number of VUMC's patients live in Middle Tennessee.
