Use the EUEUS form, in addition to the International Shipment Reviewsurvey, to document intent to ship export-controlled commodities (whether bound for a domestic or an international destination).
An EUEUS may also be used for documentation and approval purposes for any shipment with a value of $2,500 or more, per EEI requirements. See more information about EEI applicability at the bottom of the page here.
This document must be:
completed by the End-User (Ultimate Consignee),
on the receiving institution's company letterhead,
signed and dated by an authorized official of the receiving institution, and
returned to the VUMC Export Compliance office (export@vumc.org) in PDF format prior to dispatching the shipment
