Patients with chronic pain and receiving opioids will be monitored by a clinical pharmacist and discussed at weekly team meetings with practice changes resulting in enhanced management of patients receiving prescribed opioids.
Geri-PACT panel patients receiving chronic opioids will be reviewed by the PI and Geri-PACT pharmacist, administered the Opioid Risk Tool, and provided individual counseling. Data will be collected on number of patients identified for screening, the number of patients identified at risk, the milligram morphine equivalent dosing, the number of patients with annual signed opioid contracts, CSMD review of prescription history, and the number of patients receiving naloxone kits. Semiannual chart review will be performed for all patients receiving chronic opioids.
