After July 1, 2016 

The Center for Programs in Allied Health processes transcript requests for those who completed the Medical Laboratory Science Program after July 1, 2016. Please submit a Transcript Request Form to order an official transcript.

 

Before July 1, 2016 

The Vanderbilt University School of Medicine, Office of Enrollment Services processes transcript requests for those who completed the Medical Laboratory Science Program prior to July 1, 2016. To order an official transcript, please submit a Transcript Request Form by mail, fax, or email to:
