Quality leaders from Connected Care ACOs in East, Middle and West Tennessee came together May 14 for a virtual forum to share best practices, while celebrating their hard work and success in impacting MSSP quality measure performance.
This was the first time all ACOs had convened in one place, and leaders talked about their performance improvement journeys, practice workflows and factors in success. Key highlights of PY2018 – PY2019 include:
• West TN Medical Group improved fall risk screenings by 17.6%.
• NorthCrest Physician Services improved poor diabetes A1c control by 5.64%.
• Internal Medicine Clinic improved breast cancer screenings by 32.74%.
• Vanderbilt Medical Group improved influenza immunizations by 14.74%.
• Erlanger is improving tobacco use screenings and cessation interventions
• Williamson Medical Group and West Tennessee Medical Group are having success at controlling hypertension.
Sharing important lessons learned, challenges addressed and best practices will help our ACOs to continue improving care delivery transformation for patients, families and communities. With such great dialogue during the forum, similar events may be planned for the future.
