ASLS Course Approval Request

*Please note: This is for Advanced Stroke Life Support Courses, not ACLS.

 

Review the course requirements and complete the form below to request approval for your course. You will receive an email confirming or denying the request within 3 business days of submission.


Course Requirements

Course Director:

  • An ASLS Course Director must be present for all ASLS courses.

Equipment:

  • Rental equipment is not available.

Paperwork/eCards:

  • Before eCards can be issued, all required paperwork must be received by scanning and emailing to deborah.e.haight@vumc.org or turning in to the Resuscitation Program office.
    • Excel eCards file filled out with course date, names, and email addresses and emailed to deborah.e.haight@vumc.org
    • Course roster
    • Student Online Completion Certificates
    • Skills testing sheets (stroke scale checklist, part A checklist, part B checklist, part C checklist)
    • Evaluation
  • If an assisting instructor is not aligned with the Vanderbilt AHA Training Center, a copy of their current ASLS Instructor card must be submitted.
  • eCards will be issued within 20 days of receipt of all requirement paperwork. Average processing time is 1-2 weeks.
