If you are a Vanderbilt faculty or staff member and you have an injury caused by your work, the Occupational Health Clinic is here to help you. You do not have to schedule an appointment to be evaluated for a new work injury.
- If you work on the main Vanderbilt campus, come to Suite 640 of the Medical Arts Bldg. at 1211 21st Avenue South anytime on weekdays from 7:00 a.m until 4:30 p.m. After these hours and during holidays or weekends, you should call the on-call provider at 615-936-0955 for instructions.
- If you work away from the main campus, call us at 615-936-0955 for instructions. You can also call this number to speak to our on-call Nurse Practitioner after hours.
- Persons who have a life threatening injury should be transported to the nearest Emergency Service. Call 1-1111 for response to a medical emergency on the medical center campus. Call 911 for emergency transport for all other areas on campus and off campus.
- Report the injury to your supervisor and the Office of Risk and Insurance Management by completing the Tennessee First Report of Injury form (VUMC) and Tennessee First Report of Injury Form (VU).
The Vanderbilt Occupational Health Clinic serves only Vanderbilt faculty and staff.
